החלפת מפרק ירך או מפרק ברך הם בין הניתוחים האורתופדיים המסובכים והיקרים שיש, אך הינם די נפוצים. 7-10% מהניתוחים הנ"ל מדי שנה בארה"ב הינם למעשה ניתוח חוזר בגלל זיהום המפרק המלאכותי (Prosthetic Joint Infections=(PJI, מצב שלא רק מתאפיין בנכות מסויימת וכאבים עזים, אלא אף במצב קיצוני יכול להביא לספסיס – זיהום כללי בגוף ואף סכנת חיים.
ברוב הניתוחים למפרקים שהזדהמו התגלה כי החיידק העיקרי שגרם לזיהום הינו סטפילוקוקוס, חיידק שאינו אופייני לאיזור הפה. יחד עם זאת, בחלק קטן מהמקרים נמצא גם סטרפטוקוקוס, שהינו כן חיידק שכיח בפה. נשאלת השאלה האם כדאי לפני טיפול שיניים לתת מינו מיוחד של אנטיביוטיקה, בכדי למנוע זיהום מהפה אל המפרק המלאכותי (אנטיביוטיקה פרופילקטית).
כאמור בישראל ובעולם אין קוצנזוס מובהק, ואין הוראות או פרוטוקול טיפול. המוגש כאן הינו בגדר המלצות, ומורכב מסיקרה ספרותית ענפה. וברגע שמשרד הבריאות בישראל יקבע נייר עמדה, יש לעקוב אחריו.
